INSTRUCTIONS: This petition web form is used to file any one of the following 5 case types. You must submit a separate web form for each election case.

RC — CERTIFICATION OF REPRESENTATIVE — A majority of the employees in the proposed bargaining unit wish to be represented for purposes of collective bargaining by Petitioner, and Petitioner desires to be certified as exclusive representative. FOR RC ONLY: (A Showing of Interest by 30% or more of the employees in the unit must be emailed separately within 48 hours of filing this web form). RD — DECERTIFICATION — A majority of the employees in the bargaining unit claim that the certified or currently recognized bargaining representative is no longer their representative. For RD Petition use separate RD Form and email to [email protected]

RM — REPRESENTATION (EMPLOYER) — One or more individuals or labor organizations have presented a claim to Petitioner to be recognized as the representative of employees of Petitioner.

SD — SELF-DETERMINATION — Multiple units represented by the same labor organization and same employer seek to be represented in one unit. (No showing of interest required.)

UC — UNIT CLARIFICATION — Petitioner seeks clarification of the Employer's placement of a newly created (or substantially changed) position(s). (A petition for unit clarification does not raise a question concerning representation and cannot be used where an RC or RC petition is appropriate).
