Michigan Department of Labor and Economic Opportunity FOIA Request

Michigan's Freedom of Information Act (FOIA), MCL 15.231 et seq, allows certain persons to receive copies or make inspections of most public records of public bodies upon written request. The person must sufficiently describe the records he or she seeks.

A fee may be charged to cover the cost of complying with a FOIA request.

The Michigan Department of Labor and Economic Opportunity (LEO) is comprised of multiple divisions and agencies. Please review the options below carefully and select the division most relevant to your FOIA request. Each division has its own designated FOIA Coordinator who will respond accordingly.

Use this form only to request public records under FOIA. FOIA requests for confidential unemployment insurance (UI) records will be denied. To request your own UI records, complete Form UIA-6102, Authorization to Release Confidential Information and follow its submission instructions. Authorized representatives or attorneys may submit a written request for confidential UI records on letterhead along with a notice of appearance specifying the records and time period of the request, or have the claimant/employer being represented complete and legibly sign Form UIA-6102 .
