Michigan Rehabilitation Services (MRS) Inquiry Form

This form is used to submit questions, inquiries and comments to Michigan Rehabilitation Services (MRS). Information submitted on this form will be sent to a customer service representative at Michigan Rehabilitation Services who will review the submission information and respond if requested.

Do not submit sensitive personal information using this form, such as Social Security Numbers, financial payment information, or medical records.

MRS is a public agency within the Michigan Department of Labor and Economic Opportunity (LEO) responsible for the administration of Michigan’s public vocational rehabilitation program. MRS is funded 78.7% with USDOE-RSA Title I federal funds, and 21.3% with state and local funds.
